  When you first load into the new Lake Foscari map, press pause. You will see a question mark on the map - go to this location.
  
  There will be a coffin surrounded by enemies when approached. Kill the enemies and then step directly on the coffin. You will unlock the first new character, Eleanor. Upon quitting or completing your first run after doing so, the achievement will unlock.

  I recommend using Queen Sigma to make life easier. You may also wait until minute 25 to defeat the boss in this map for another achievement.

  Uh oh. Got the coffin, unlocked Uziron, but no achievement...
  
  Edit: As said below, quit the game and go to your collection and it should pop!

  I can't even find a way to get to the coffin, i keep hitting barriers (either trees, blue plasma, or blackness). Is there a certain route to get there?
  
  EDIT: Nevermind, just found a bridge to help me cross the blue plasma. This level isn't as straight forward as the previous ones.
  
  2nd EDIT: Achievement didn't pop for me either. Back to the main menu and should've gotten the achievement for the Coffin and the Boss at 25:00. The results screen said "2 achievements unlocked" but the Xbox itself didn't pop them, and I'm not showing them popped in my history (no silent pops).
  
  3rd EDIT: I hard-quit the game (pressed Start on the Home Screen and "Quit" on the game), loaded back in, went to "Collection", and the 2 achievements popped. Try that if you didn't get it on your run.
