You unlock Pugnala by finding and opening the coffin in the Mad Forest by defeating the ring of enemies that spawns when you approach it. Once you have her, take her into any level and survive for 10 minutes. Il Molise is the easiest one to do this in, as none of the enemies in that level actually move.
